Sailor Soldiers & Allies
This contains a list of the Sailor Soldiers & their Allies (in both civilian and Soldier form).
Each character has information about possible theories behind the writers' changes.
 
Usagi Tsukino vs. Serena Tsukino

T: They changed Usagi's name to Serena, because it is a pun on Selene, who was the goddess of the moon. This was done so they could still have a connection to the Moon without using her Japanese name. The name Usagi Tsukino, was a play on words being the princess of the moon, since the Japanese see a 'rabbit of the moon'. They avoided giving Serena a last name.
SMS: Serena's name was kept the same from the first 2 seasons, but her nickname, which was "Meatball Head" in season 1 and 2, would change from 'Meatball Head' then to 'Moon Face'. Don't ask me why. Either way, it was a writer inconsistency that was made throughout the season. Amara would call her 'Moon Face', even though she didn't know that Serena was Sailor Moon at this stage.
SMSS: After 3 seasons of avoiding this, they finally decided to give Serena a last name. They even used her original Japanese last name Tsukino! Her nickname has gone back to Meatball Head, like in season 1 & 2. It was better than Moon Face.

T: They changed Serenity to Serena, so that it would keep a link between Serena and the Moon Princess. This maybe would've been done for the younger viewers of the show.

T: No changes made to this character's name. Although when Sailor Moon transformed into her in the final battle, Sailor Venus referred to her as "Queen Serenity".
   
Ami Mizuno vs. Amy Mizuno

T: The Japanese name 'Ami' is pronounced [Ah-Me] and 'Amy' [Aye-Me]. I would say they did this to make it similar to the original name, but to be pronounced in a more American tone. Ami was given a different English last name to the original to make her sound more Americanized, but I have no idea where they got Anderson from. The writers probably just thought of it on the spur of the moment.
SMS: Amy's name was kept the same from the first 2 seasons, but was now given a new last name, 'Mizuno'. 'Anderson' being her last name for the first season dubbed by DiC. Cloverway's writers obviously didn't have much knowledge of the first season. But, it's good to see the original last names starting to surface.

Rei Hino vs. Raye Hino

T: They changed 'Rei' to 'Raye' so that it would have a more American look to it. Raye was never given an official English last name in this season.
SMR: They kept her first name the same from season 1. Finally Raye was given a last name and they kept the original, 'Hino' and they also did a pretty good job at pronouncing it.

Makoto Kino vs. Lita

T: The writers derived the name 'Lita' from 'Leda', which is one of the moons of Jupiter. Also 'Leda', in Greek Mythology was the wife of Zeus, which Jupiter is the Roman equivalent. Either of the two could be a possible reasons for how the name 'Lita' came about. Lita was never given an official English last name during the series.

T: Mina can be used as an English name, and which also happens to be the nickname that Artemis called her in the original Japanese series. Mina was not given an official English last name in this season.

Chibiusa vs. Rini

T: Her name was changed to suit the American re-naming system, but they never mentioned Rini being her nickname, but as being her full first name. Rini is a shortening of Serena, which is fitting for her character in keeping the original Usagi/Chibiusa crossover. A last name was not given, but they kept her kingdom name, Small Lady.
SMS: Rini's name was kept the same from season 2, but unlike season 2 instead of keeping her original kingdom nickname "Small Lady", it was changed to "Little Lady", this could have just been a bad translation on the writer's behalf.

J: Setsuna Meiō 「冥王せつな　Meiō, Setsuna」

T: The first name was changed to suit the rest of the American names. I'm guessing they chose Trista because of its English origin which means 'The melancholy one'; a related trait of Setsuna's personality. Setsuna's last name was kept and the same system (Meioh) that Naoko Takeuchi used for the manga profiles was also used for the English adaptation.
   
Haruka Ten'ō vs. Amara Tenoh

T: The first name was changed to suit the rest of the American names, but I am not sure how they got Amara from Haruka. Amara is Greek for 'unfading' and Haruka is a pun on the Japanese word for 'distant'. Haruka's last name was kept and the same system (Tenoh) that Naoko Takeuchi used for the manga profiles was also used for the English adaptation.

T: In Greek, Darien means 'wealthy' and in Spanish it means 'a placename'. I am unsure of how Mamoru became Darien given the meaning of the name, but even so, it removes the pun that his original Japanese name intended. Mamoru is a pun on the Japanese word meaning 'to protect', or 'to guard', and his last name which uses the same 「地　chi」 kanji character that is used for the Japanese word for the Earth 「地球　Chikyū」. Darien was not given an official last name in the series, although his last name was "Shields" in the Scout Guides and English manga, and "Chiba" (original Japanese) in the Resource book for the Sailor Moon Role Playing Game.

T: The transition from Prince Endymion to Prince Darien was done to keep a link between civilian Darien and the Prince of the Earth. This would've been done to make it less confusing for the younger viewers, BUT in doing so, they trap themselves in the episode when Serena talks to Evil Prince Darien and "Prince Darien" states he has never heard the name "Darien"... but wait... isn't you're name Prince Darien??!! *stares blankly*

T: The English dub looks to have used the Romanization of his Japanese name Perle, which is written as Peruru. This was obviously a mistake made by the writers of the SuperS movie, who didn't take the time to learn about the origin of the name.
